The physician acts as a coach or guide to empower the patient and the patient’s family to take personal responsibility for each member's health. A continuing dialog between the team partners allows the patient to become fully informed about his or her health, options for treatment, and the risks and benefits of those options.

Individualized plans promote integrated health and healing for each patient by incorporating physical, mental, emotional, and spiritual therapies. The partners develop a health-promotion plan that focuses on wholeness and health rather than symptoms or disease "labels". More than "quick fixes" that may suppress symptoms or distract from the deeper issues, health-promotion solutions will aim for healing the entire person on all levels.
